Helping patients manage chronic conditions typically falls to primary care providers or specialists. At the same time, those people are just as likely as other patients to walk into your urgent care center—if not more so. CityMD and Mount Sinai Health System in New York City have teamed up to formalize a referral system that is designed to benefit all parties concerned. Mount Sinai will encourage patients to visit CityMD clinics when they have a need for immediate care for non–life-threatening complaints, and CityMD physicians will have established channels for ensuring patients’ regular providers are up to date on care provided in the urgent care setting. Both parties will utilize quality metrics for the shared provider network, as well as EMRs so patient data can be accessed faster. Mount Sinai has about 7,100 primary and specialty physicians throughout New York City and surrounding areas, as well as in Florida, and 31 affiliated community health centers. CityMD has more than 50 locations in the New York metropolitan area.
