Diagnosis: This x-ray shows a suspicious line at the base of MC1, which could be a fracture. The fact that there is no history of trauma does not rule this out, because it may be a pathological fracture. The whole area around the base of MC1 is very ragged (arrows) and is consistent with osteomyelitis. This patient should have an immediate workup to rule out osteomyelitis.
