The diagnosis is fracture of the radial head.
Next steps: if there is significant pain, cast splint and follow-up with an orthopedist or sling for 7-10 days, start range of motion, and re-evaluate in urgent care. If pain is not significant, elastic wrap and sling and follow-up.
