Introduction
Dyspnea is the subjective sensation of difficult, uncomfortable, or labored breathing. Causes range from benign and self-limited conditions to immediately life-threatening cardiopulmonary, upper-airway, toxicologic, metabolic, and neurologic disorders. In urgent care, the first task is not to establish a complete differential diagnosis; it is to identify physiologic instability and conditions that require immediate treatment and emergency transfer.
Assessment and stabilization should occur simultaneously when the patient appears ill. A normal oxygen saturation, normal lung examination, or absence of chest pain does not by itself exclude serious disease.
Presentation
- Shortness of breath or air hunger
- Chest tightness or pressure
- Inability to take a satisfying deep breath
- Choking or throat-tightness sensation
- Wheezing or noisy breathing
- Cough
- Reduced exercise tolerance
- Orthopnea or paroxysmal nocturnal dyspnea
- Frequent sighing or hyperventilation
- Visible respiratory distress
Triage Pearls
- Patients who appear to be in respiratory distress should bypass routine intake and be assessed immediately by clinical staff.
- Obtain respiratory rate, heart rate, blood pressure, temperature, and pulse oximetry promptly. Repeat abnormal values and assess trends.
- Treatment should not be delayed while the precise cause of severe dyspnea is being determined.
- Call EMS early for severe respiratory distress, altered mental status, cyanosis, hemodynamic instability, suspected upper-airway obstruction, or another time-sensitive emergency.
- Do not rely on pulse oximetry alone. FDA guidance emphasizes that readings can be affected by poor perfusion, motion, nail products, skin pigmentation, and other factors; oxygen saturation may be overestimated in some patients with darker skin pigmentation.1

History Pearls
Focus the history on features that change immediate management.
- Onset: sudden vs gradual; exact time of onset; first episode vs recurrent.
- Course: intermittent, persistent, progressive, exertional, positional, or nocturnal.
- Associated symptoms: chest pain or pressure, palpitations, syncope/presyncope, fever, cough, sputum, wheezing, hemoptysis, orthopnea, edema, choking, stridor, rash, facial/oral swelling, neurologic symptoms.
- Cardiopulmonary history: asthma, COPD, heart failure, coronary disease, arrhythmia, valvular disease, prior pneumothorax, pulmonary hypertension, or interstitial lung disease.
- Venous thromboembolism risk: prior DVT/PE, active cancer, recent surgery or immobilization, long travel, estrogen exposure, pregnancy/postpartum state, unilateral leg symptoms.
- Infectious exposure: recent respiratory illness, influenza/COVID-19 exposure, immunocompromised state.
- Environmental/toxic exposure: smoke, combustion sources, occupational irritants, chemicals, or multiple similarly ill household members. A normal conventional pulse oximeter does not exclude carbon monoxide poisoning.2
- Medication history: bronchodilator use, diuretics, anticoagulants, beta-blockers, ACE inhibitors, sedatives/opioids, and recent medication changes.
- Smoking/vaping: current and previous tobacco exposure, vaping, or inhalational drug use.
Physical Exam Pearls
- Assess appearance, mental status, ability to speak, respiratory effort, accessory muscle use, retractions, and position.
- Count the respiratory rate rather than relying only on an automated value.
- Interpret oxygen saturation in clinical context and compare with the patient’s known baseline when available.
- Examine for stridor, voice change, oral or tongue edema, drooling, and other signs of upper-airway compromise.
- Listen for wheezing, crackles, focal bronchial breath sounds, diminished or asymmetric breath sounds, and prolonged expiration.
- Assess heart rate/rhythm, murmurs, gallop, jugular venous pressure, peripheral perfusion, and edema.
- Inspect the legs for unilateral swelling, tenderness, or asymmetry when VTE is a consideration. Homan’s sign should not be used to diagnose or exclude DVT.
- In children, look for age-specific tachypnea, retractions, nasal flaring, grunting, poor feeding, apnea, lethargy, and cyanosis.
Testing Pearls
Testing should be driven by the clinical syndrome. A critically ill patient should be stabilized and transferred rather than kept in urgent care to complete a diagnostic workup.

Troponin, BNP/NT-proBNP, blood gas analysis, CT pulmonary angiography, formal echocardiography, and advanced cardiopulmonary testing are generally hospital-level investigations. Some urgent care centers may have selected laboratory capabilities, but testing should not delay transfer when ACS, PE, acute heart failure, respiratory failure, or another time-sensitive diagnosis is suspected.
Diagnosis Pearls
Dyspnea is a symptom, not a diagnosis. More than one process may be present. The urgent care clinician should first identify immediately dangerous causes, then determine whether a stable patient has a syndrome that can reasonably be treated and followed as an outpatient.

Asthma and COPD
Wheezing, prolonged expiration, chest tightness, and a known obstructive lung disease history support asthma or COPD, but wheezing is not specific. Severe work of breathing, altered mental status, hypoxemia, exhaustion, or markedly diminished air movement should be treated as a respiratory emergency. Current GINA and GOLD guidance supports rapid bronchodilator therapy and early systemic corticosteroids for clinically significant exacerbations, with escalation or transfer when the response is inadequate or severe features are present.9,10
Pneumonia and Viral Respiratory Infection
Fever, cough, focal lung findings, hypoxemia, tachypnea, or systemic illness should raise concern for pneumonia. Chest radiography is appropriate when the diagnosis is uncertain, when abnormal vital signs or focal examination findings are present, or when complications are possible.4,11 Dyspnea due to influenza, COVID-19, RSV, or another viral infection may coexist with bacterial pneumonia, asthma/COPD exacerbation, or cardiopulmonary disease.
Pneumothorax
Consider pneumothorax with sudden dyspnea and unilateral pleuritic chest pain, especially after trauma or procedures, in patients with underlying lung disease, or in those with a prior spontaneous pneumothorax. Unilateral reduction in breath sounds increases suspicion. If tension physiology is suspected, immediate emergency management and EMS transport take priority over diagnostic imaging.
Pulmonary Embolism
PE remains an important consideration in unexplained acute dyspnea. Oxygen saturation and chest radiography may be normal. Use a validated pretest-probability approach rather than ordering D-dimer indiscriminately.
- PERC: In adults already judged to have a low pretest probability for PE, the Pulmonary Embolism Rule-out Criteria may identify patients who require no further PE testing when all criteria are negative: age <50 years, pulse <100/min, oxygen saturation at least 95%, no unilateral leg swelling, no hemoptysis, no recent surgery/trauma requiring hospitalization, no prior DVT/PE, and no estrogen use.6
- D-dimer: If PE is not excluded by PERC and pretest probability remains low or intermediate, a high-sensitivity D-dimer can be used. In patients older than 50 years, ACEP supports an age-adjusted threshold (age x 10 ng/mL FEU when the standard assay cutoff is 500 ng/mL FEU).6
- Transfer: Patients with high clinical probability, hemodynamic instability, significant hypoxemia, or a positive D-dimer that requires definitive imaging should be evaluated in an ED capable of CT pulmonary angiography or alternative definitive testing.
PERC should not be applied to patients who are not already low risk. Do not allow a risk score to override concerning clinical findings.
Acute Coronary Syndrome
Dyspnea may be an anginal equivalent, particularly in older adults, women, and patients with diabetes. Obtain an ECG promptly when ischemia is possible. Current ACC/AHA guidance recommends that patients with clinical evidence of ACS or another life-threatening cause be transported urgently to the ED, ideally by EMS, and specifically cautions against delaying transfer from an office setting to obtain troponin or other diagnostic testing.3
Acute Heart Failure
Orthopnea, paroxysmal nocturnal dyspnea, edema, JVD, crackles, and pulmonary vascular congestion support heart failure but are not individually diagnostic. Chest radiography can identify congestion and alternative diagnoses but has limited sensitivity. Where expertise is available, lung POCUS showing diffuse bilateral B-lines can improve diagnostic accuracy when integrated with the clinical assessment.7,8,12 New or significantly decompensated heart failure with hypoxemia, chest pain, hypotension, severe hypertension, arrhythmia, or substantial respiratory distress warrants ED evaluation.
Upper-Airway Emergencies
Stridor, drooling, muffled voice, tongue/oropharyngeal swelling, tripod positioning, or rapidly progressive throat symptoms should be treated as potential airway emergencies. Suspected anaphylaxis requires prompt intramuscular epinephrine; airway and cardiovascular compromise require EMS transport.13 Suspected epiglottitis or deep neck infection should not be managed as routine pharyngitis, and unnecessary airway manipulation should be avoided while arranging emergency evaluation.
Carbon Monoxide and Other Toxic Exposures
Consider carbon monoxide poisoning when dyspnea, headache, dizziness, nausea, chest symptoms, or altered mental status occur after combustion exposure or when multiple people from the same environment are ill. Conventional two-wavelength pulse oximetry cannot reliably detect carboxyhemoglobin; suspected poisoning requires appropriate oxygen therapy and confirmatory co-oximetry, usually in an ED.2
Hyperventilation and Anxiety
Anxiety and hyperventilation can cause marked dyspnea, chest tightness, paresthesias, and lightheadedness, but they are diagnoses of exclusion in a new or atypical presentation. Do not attribute dyspnea to anxiety until dangerous cardiopulmonary, metabolic, and toxicologic causes are reasonably excluded.
Treatment and Disposition Pearls
Treatment should be directed at the suspected cause while avoiding delays in transfer. Supplemental oxygen is a treatment for hypoxemia, not for the sensation of breathlessness alone. For most acutely ill adults who require oxygen and are not at risk for hypercapnic respiratory failure, commonly recommended target saturation is approximately 94%-98%. For patients with COPD or another risk for hypercapnic respiratory failure, a target of 88%-92% is generally recommended pending blood-gas assessment and higher-level evaluation.14

Medicolegal Pearls
- Document the patient’s appearance, respiratory effort, complete vital signs, oxygen saturation, relevant repeat measurements, and response to treatment.
- When PE is considered, document the pretest-probability assessment and the specific rule or pathway used. Do not document a Wells or PERC score unless the components were actually assessed.
- Document why an emergent diagnosis was considered reasonably excluded, or why transfer was recommended when it could not be excluded.
- Do not allow normal oxygen saturation, a normal chest radiograph, or an initially normal ECG to provide false reassurance when the history or overall clinical picture remains concerning.
- When imaging is performed, establish a reliable process for final radiology interpretation and follow-up of discrepant or incidental findings.
